Honestly for me it wouldn't even need to be a mechanical penalty. Just have >insert place/person< treat me differently. So I guess missing out on rewards, or being expected to give something up works.

Like, failing to pull off a job for a 'crimeboss' npc leads to him crippling me or an npc.
